摘 要 目的:探讨慢性牙周炎患者龈下菌斑中牙龈卟啉菌(porphyromonas gingivalis,Pg)和伴放线放线杆菌(actinomycetemcomitans actinobacillus,Aa)所携带的不同基因与临床症状间的关系。方法:采用PCR对66位患者127例龈下菌斑中的Pg 16SrDNA基因、prtC基因、fimA基因以及Aa 16SrDNA基因、1ktA基因和fap基因进行检测;并对上述基因的检出与牙周袋深度、牙槽骨吸收程度、松动度以及牙龈指数(gingival index,GI)和菌斑指数(plaque index,PLI)间的关系进行了统计分析。结果:在127例龈下菌斑中,Pg 16SrDNA、prtC和fimA的阳性率分别为90.6%、81.9%和78.0%,Aa 16SrDNA、1ktA和fap的阳性率分别为92.1、81.1%和29.9%。fimA阳性率与牙周袋深度有关(P=0 .004),在大于7 mm的牙周袋中阳性率最高(P<0.05)。Pg prtC、fimA和Aa fap的检出率都与GI有关(P<0.05)。Aa fap阳性率与PLI有关(P=0.002)。结论:fimA在较深的牙周袋中更易检出。Pg partC和Aa fap可能与轻度炎症有关。 关键词 慢性牙周炎 龈下菌斑 牙龈卟啉菌 伴放线放线杆菌 聚合酶链式反应 症状
Relationships between clincal symptoms and detection of different genes of Porphyromonas gingivalis and Actinomycetemcomitans actinobacillus in subgingival plaques Wu Yanmin,Chen Lili,Yan Jie,et al.the 2nd Affiliated Hospital,College of Medical Sciences,Zhejiang University,Hangzhou 3100009,China Abstract:Objective:To find the relationships between clinical symptons of chronic periodontitis and frequencies of different genes of Porphyromonas gingivalis (Pg) and Actinomycetemcomitans actinobacillus in subgingival plaques.Methods:Polymerase chain reaction assays were used to detect Pg 16SrDNA gene,collagenase gene (prtC),fimbrillin gene (fimA),Aa 16SrDNA gene,leukotoxingene (lktA),and fimbriae associated protein gene (fap) in 127 subgingival plaque samples from 66 chronic periodontitis patients.The relationships between the detection rates of these genes and clinical parameters such as pocket depth,alveolar bone resorption,tooth movement,gingival index (GI) and plaque index (PLI) were statistically analyzed.Results:In 127 subingival plaque samples,the positive rates of Pg 16SrDNA,prtC,fimA,Aa 16SrDNA,lktA and fap gene detection were 90.6%,81.9%,78.0%,92.1%,81.1% and 29.9% respectively.The positive rate of fimA was highest when pocket was deeper than 7 mm (P<0.05).The positive detection rates of prtC,fimA and fap were found to be associated with GI (P<0.05).The posi tive rate of fap has something to do with PLI (P=0.002).Conclusion:fimA genewas more easily found in deep pockets.prtC and fap may play a role in slight inflammation of periodontal tissue. 通常认为牙龈卟啉菌(porphyromonas gingvialis,Pg)和伴放线放线杆菌(actinomycetemcomitans actinobacillus,Aa)是慢性牙周炎(chronic periodontitis,CP)的主要病原菌。在龈下菌斑中检测Pg和Aa对于牙周炎的诊断、疗效判断以及预防等方面均有重要意义。Pg和Aa临床分离菌株毒力基因的携带及表达情况存在差异[1,2],故检测Pg和Aa毒力基因有助于阐明Pg和Aa的致病机理。16SrDNA基因具有高度保守性和种特异性,故常在PCR中作为不同种类细菌鉴别的靶基因。本文采用PCR方法检测了Pg16SrDNA基因、胶原酶基因(prtC)和菌毛基因(fimA),以及Aa 16SrDNA基因、白细胞毒素基因(lktA)和菌毛相关蛋白基因(fap),并初步探讨了上述基因的检出与患者临床症状间的关系。
